To : All Government Officers

SUBJECT : SECURITY CHECKS ON GOVERNMENT OFFICERS

The attention of all Government Officers is drawn to the confidential memorandum on the above subject issued by the Colonial Secretary's Office on the 8th May, 1956.

2\. As a reminder, all Government Officers are required to fill in MI 104A form in duplicate when applying for a new post or when their duties are changed substantially. The completed forms should be submitted to the appropriate authority as indicated in paragraph 4 of the memorandum.

3\. In order to facilitate the checking procedure, it is essential that the forms should be filled in completely and accurately. Any delay in the submission of the forms or incomplete information furnished will cause unnecessary delay in the processing of the applications.

4\. The following particulars should be carefully checked before the forms are submitted:

Nationality By birth or by naturalization

5\. It is the responsibility of every Government Officer to ensure that the information given is correct and up-to-date. Any failure to comply with this requirement may result in serious consequences.
